Sign up for the Automated Payment Program
Please send this form and either a copy of a voided check or a letter from the bank with your new account and routing numbers. Please email to [email protected] 
or mail to 132 N. Elmwood Ave. Medina, Oh 44256, or bring to Medina City Hall.  The first month will be a Pre-Note to verify all the information, then the following month the payment will be taken from your account.
