Please send this form and either a copy of a voided check or a letter from the bank with your new account and routing numbers. Please mail to 132 N. Elmwood Ave. Medina, Oh 44256, or bring to Medina City Hall, or use the night drop box located at 135 N. Elmwood Ave.  The month after entry, the payment will be taken from your account.
